Good discussion can create clarity in the moment, but memory is a weak operating system. A useful engagement leaves behind a concise record that another responsible person can read and act on without recreating the entire meeting.

Capture the situation, decision and work

The situation brief records the challenge and relevant context. The decision record shows what was chosen, which alternatives were considered and which assumptions remain open. The action plan turns that decision into owners, actions and review timing.

  • Keep the situation brief short enough to reread before the next review.
  • Label unresolved questions instead of smoothing them over.
  • Assign one accountable owner to each next action.

Design the output for follow-through

The format should match the work. A one-page decision record can be more useful than a long presentation when the immediate need is alignment. The test is simple: can the team see what changed, what happens next and when the decision will be revisited?
